I think you have to define "success" to answer (1).
If you judge based on the stated reasons for the military actions of NATO, from Serbia to Syria, you're correct that none resulted in peaceful western style democracies.
If you judge based on removing potential rivals to US global financial hegemony, removing or curtailing the power of US adversaries and their allies (particularly in the Mid East), the profits of US weapons manufacturers, AND you discount the pain, suffering, death and massive financial losses to the non-NATO people and countries, then the record looks pretty good.
